Submission of this form meets the requirements of the Prescription Donation Repository Program established by the Kevin Clauson Drug Donation Act, Tennessee Code Annotated, Title 63, Chapter 10. All participants in the program must act in good faith and agree to make donations in accordance with the Act.
This form automatically filters for eligible medication donations in accordance with Tennessee law and our internal pharmacy safety standards, which prioritize the safety of the patients we serve.
Eligible donations are unopened, unexpired, sealed in the original manufacturer’s packaging, and in the form of capsules, tablets or inhalers. Blister packs are eligible, as long as unused doses remain sealed.
Donations may be ineligible if they are loose pills (even those in pharmacy-issued brown bottles); expired, or have less than six months before expiration; require refrigeration; controlled substances, as designated by the U.S. Food and Drug Administration; birth control; REMS drugs as classified by the U.S. FDA; or in a dosage form not currently eligible per Tennessee state law and RemediChain’s safety protocols.
Although we currently can only rescue capsules, tablets, and inhalers, we hope to expand to other dosage forms in the future.
